CCA GUJARAT INSTRUCTIONS FOR SUBMISSION OF DLC
It is to inform that the validity of Life Certificate furnished by you is valid only up to 31/10/2021. Pension for the month of November-2021 and onwards could be drawn and disbursed only on submission of a valid Life certificate. Hence you are hereby requested to furnish the Life Certificate (LC)/Digital Life Certificate (DLC) to the O/o CCA Ahmedabad (Do Not submit to Bank), through any one of the following options:
(i) The pensioner’s lounge functioning at the premises of “O/o CCA, 4th Floor, P & T Admin Building, Khanpur, Ahmedabad” and BSNL Circle office, JTO (Rectt), B-wing 3rd Floor, CG Road, Ahmedabad.
(ii) Post Office IPPB service at your home. Download Postinfo App from Google Play store, Go to Service Request, submit Name Address Pincode Mobile Number, Select service as IPPB and then Jeevan Pramaan, after confirmation of OTP, Postman/GDS will visit your home for DLC (Service charges applied)
(iii) At Common Service Centres (CSC) located in the village panchayats/Seva sadan with applicable charges.(https://jeevanpramaan.gov.in/locatecentre)
(iv) Through the Banks (v) Physical Life Certificate duly signed by the authorities as defined in Rule 343 of Central Treasury Rules (CTRs), could be submitted by post or by hand, to the office of C.C.A. 4th Floor, P & T Admin Building, Khanpur, Ahmedabad 380001”.
